The Department also contacted the Operator in 1/15/2015 to inquire about the current condition of the site. As on 1/17/2014 the Department has not received a return phone call from the Operator.
2257032 Primary Facility 2014-03-25 Follow-up Inspection Outstanding Violations - No Viols Req'd At the time of inspection the Operator has failed to respons to violations written on 1/17/2014. The Deparment set a deadline of 2/25/2014 for a respopnse, at the time of inspection no response received from the Operator. No change in site condition. Pit resumes open onsite and site has not been restored.
2513841 Primary Facility 2016-08-12 Routine/Complete Inspection Violation(s) Noted Department records indicate well was permitted on 1/2/2008 and expired on 1/3/2010. At the time of inspection, site construction appears to have commenced, as indicated by what appears to be an access road and unlined drill pit. Well not drilled and site has not yet been restored. Construction equipment present on site during inspection. Per current regulations if a well site is constructed and the well is not drilled, well site shall be restored within 30 days after the expiration of the well permit.

Department requests Operator follow-up with Department, in writing upon investigation of reported concerns identifying cause of violations, what Operator has done and will do to correct situation and achieve compliance and schedule/time frame of anticipated compliance activities.

Department records indicate exisiting outstanding violatons under 25 Pa. Code § 78.65(2) dated 1/17/2014 (Insp. ID# 2237700 and Enforcement ID# 306689).

2519004 Primary Facility 2016-09-09 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Inspection performed as part of a follow-up to 8/12/2016 inspection (Insp.# 2513841 Enf.# 346338). At the time of inspection site inaccessible due to the presence of construction equipment on access road. Site restoration work appears to be ongoing. Department will conduct follow-up inspections to monitor site restoration progress.

2544437 Primary Facility 2016-11-30 Compliance Evaluation In Compliance with Policy Inspection was performed as a Compliance Evaluation to 8/12/2016 inspection (Insp.# 2513841 Enf.# 346338). At the time of inspection unlined drill pit has been back-filled, site regarded and seeded. Minimum 70% perennial vegetative growth has not been achieved yet. Department recommends Operator continue to monitor site to ensure perennial growth is achieved and site stability is maintained. Department will conduct follow-up inspection to ensure vegetative growth is achieved.

Previously issued violations have been corrected.

2589750 Primary Facility 2017-05-03 Routine/Complete Inspection No Violations Noted At the time of inspection site has been restored. Vegetative cover has begun to establish, however, 70% uniform vegetative cover has not been attained. Operator informed Department that land is going to be tilled and cultivated. Department requests Operator provide a written statement indicating that the landowner is going to farm the land.
