Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1821665 Primary Facility 2009-08-12 Drilling/Alteration No Violations Noted US ENERGY DRILLING FOR HUNTLEY, I ARRIVE ON LOCATION WITH MIKE ARCH, FOUND ACCESS ROAD IN BAD SHAPE, PIT WALLS ARE STRAIGHT UP AND DOWN WITH NO FENCE AROUND PIT. E&S PLAN IS NOT SITE SPECIFIC. ERIC FINDLEY AGREED TO TAKE CARE OF ISSUES. I WILL RETURN TOMORROW TO OBSERVE PROGRESS
1821679 Primary Facility 2009-08-18 Drilling/Alteration No Violations Noted US ENERGY RIGGED BACK UP TO DRILL. HUNTLEY DECIDED NOT TO PRODUCE THE OPEN FLOW AT 1410. THEY CONTINUED TO DRILL UNTIL THEY HIT 100GPM, NOW THEY HAVE WATER TRUCKS RUNNING CONSTANTLY TO KEEP UP WITH THE WATER. FINDLEY SAYS HE CAN SUBSTITUTE HIS BOP AS A DIVERTER ONLY WITHOUT VOLATING 78.72. I INTERPRET THIS PASSAGE TO MEAN THAT A BOP STACK MAY NOT BE ATTACHED TO ANY LESS THAN 400' REGARDLESS OF INTENTION TO USE OR NOT
1821687 Primary Facility 2009-08-13 Drilling/Alteration No Violations Noted US ENERGY DRILLED TO 1922, SMELLED GAS BUT DID NOT REALIZE UNTIL THEY WERE TRIPPING OUT, HOW MUCH GAS THEY HAD. SUPERIOR WAS ON SITE LOGGING THE WELL WHEN I ARRIVED. DAVE JANCO & ERIC FINDLEY WERE ALSO ON LOCATION. THEY HIT 1/2 MILLION OPEN FLOW, HAVE CEASED DRILLING AND DETERMINED THE GAS TO BE COMING FROM 1410 FROM THE LOYALHANNA. ERIC HAS 1400' OF 4.5 ON IT'S WAY FROM MCJUNKEN AND AN 8 5/8-4.5 PACKER ON ROUTE FROM BIG JOHN AT MILLER'S. THEY ARE MOVING OUT THE RIG, MOVING IN A SERVICE RIG AND SETTING THE 4.5 ON A PACKER TO TAKE THE PRESSURE OFF THE 9 5/8. THEIR PLAN IS TO CLOSE IN AND MONITOR THE ANNULUS OUTSIDE THE 4.5 AND PUT IT INTO THEIR LINE (30LBS) AND PRODUCE LOYALHANNA GAS FROM 1410 WITH RATHOLE TO 1922. THEY PLAN TO APPLY FOR A 1000' VERTICAL VARIANCE, SKID ADN DRILL ANOTHER WELL NEXT TO THIS WELL, CEMENTING CASING IN FROM 2410 UP AND HAVE AGREED TO CEMENT CASING IN AT 400 ON THE NEXT WELL SO THEY CAN USE THEIR BOP IF THEY TAKE ANOTHER GAS KICK. THEIR E&S PLAN HAS BEEN UPDATED PITS ARE FENCED IN, ACCESS ROAD HAS MORE LIMESTONE
1870167 Primary Facility 2010-02-04 Follow-up Inspection No Violations Noted PRODUCING WELL, OPEN PIT, FENCE IS FALLING DOWN AND PARTIALLY MISSING. ANNULUS IS CLOSED, SITE HAS NOT YET BEEN RESTORED.
