2038108 Primary Facility 2012-01-25 Routine/Complete Inspection No Violations Noted Well was staked, drilling was not ongoing at the time of the inspection. McCready 4H well located onsite and it appared to be equipped for production. A second drill pit had been constructed onsite. The second pit had been properly lined and the contents of the first drill pit had been moved to the second pit. Second pit had more than the required two feet of lined freeboard. Phone conversation with Penneco confirmed that this was so that the first pit may be relined and reused when drilling the McCready 5H well. All access road and site erosion controls were functioning properly and there was no observed accelerated erosion or sedimenation in relation to either the access road or well-site.
2044966 Primary Facility 2012-02-23 Routine/Complete Inspection No Violations Noted n response to a spud noticed the departemnt received a Routine Completed Instepection was completed on 2/23/2012. The location of the McCrady 5H location was marked. The drill pit has been constructed and lined. There was no accelerated erosion observed in relation to the site or access road. The operator should continue to monitor the access road well pad for accelerated erosion. It was also noted that the pit liner had been compromised by numberous stakes and chain link fence posts.

2069617 Primary Facility 2012-05-21 Routine/Complete Inspection No Violations Noted At the time of inspection the well appears to drilled but not equipped for production. The drill cuttings pit onsite appeared to be in sound condition with the required amount of freeboard. The flow back pit also appeared to be in sound condition with the required amount of freeboard. The Operator noted during the inspection the well had been top holed and they were preparing for the horizontal equipment to be delivered in the following week No accelerated erosion observed in relation to the access road or well pad area at the time of inspection. The E&S plan was onsite and properly implemented and maintained.
2380333 Primary Facility 2015-06-18 Routine/Complete Inspection No Violations Noted At the time of inspection the well was drilled and equipped for production. The brine tank, pump jack and well apparatus appear to be in sound condition. The site appeared to be restored. No observation of accelerated erosion or sedimentation in relation to the well site or access road.
