2551514 | Primary Facility | 2016-12-22 | Incident- Response to Accident or Event | In Compliance with Policy | Inspection performed in response to Operator’s report of a 6bbl. “spill of diesel/oil base mud onto the primary containment at the well pad” on 12/21/2016 @ 1900 hrs. Operator informed Department that spill occurred due to the over flow of the diesel/oil base mud tank caused by the back flow of the shaker contents into the tank when worker opened shaker valves while forgetting to shut off valve into (diesel/oil base) mud tank. Operator calculated the release volume based on the recovery of the diesel/oil base mud from clean-up operations.
At the time of inspection, on-site representative stated that release occurred on to secondary containment and clean-up had been completed. Although spill does not appear to have impacted the ground at this time, but Department unable to adequately determine if the structural integrity of secondary containment has been compromised due to the presence of drilling equipment. Department recommends Operator notify Department upon the removal of the secondary containment. Depa |
