All BMPs were appeared to be installed as outlined in the erosion and sedimentation plan. The Department reviewed the PPC plan and the ER plan. The Department recommends the Operator add a containment plan during drilling activity. The Operator proposed three redline changes to the E&S plan. The Operator proposed the addition of 10' of 12" silt sox be added to the inlet side a preexisting culvert near the rock construction entrance. Redline change approved. 2618297 Primary Facility 2017-07-24 Incident- Response to Accident or Event Violation(s) Noted Department inspection in response to a release of diesel fuel reported to the Department. The Operator indicated that a dump truck carrying drill cuttings punctured the driver side fuel tank when the driver ran over the steel drive over flare stack. The Operator reported the release to the PADEP spill hot line on Friday July 21st at approximately 10:45am. According to the voicemail left with the Department approximately 60 gallons of diesel fuel was lost. During the inspection the Operator could not estimate on the amount of diesel fuel lost to the ground. The area near the flare stack was covered with matting boards and the secondary containment was partially visible. The area of the access road impacted is approximately six inches wide and 500 feet in length. The location where the dump truck stopped on the access road appears to be the most heavily impacted area. The impacted areas is approximately 10 feet wide and 30 feet in length. 2629426 Primary Facility 2017-08-25 Incident- Response to Accident or Event Outstanding Violations - Viols Req'd Follow up inspection to violations issued on 7/27/2017 related to a release of diesel fuel. The area impacted appeared to be backfilled. No sheen was observed and no stained soil was observed. The Operator indicated they would be employing the expedited method of Act 2 with a submission of a final report within 90 days. The Department will follow up with a compliance inspection after the submission and review of final report is complete.
