2126618 | Primary Facility | 2012-12-11 | Routine/Complete Inspection | No Violations Noted | During the inspection, I observed compromised liner integrity in several areas in the secondary containment liner were the frac tanks were stored at the site. I used an an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen areas of ponded water on the secondary containment liner. I recorded a conductivity reading of (2.5) milliSiemens/cm on the secondary containment liner behind the frac tanks and next to the earthen perimeter berm of the site. This location would be in the SE corner of the Well Pad. I recorded a conductivity reading of (1,200) microSiemens/cm in ponded water on the Well Pad in the SW corner of the site. A vacuum truck from Meshoppen Stone was on-site removing water both on and off the secondary containment liner. The driver of the truck informed me his plan was to remove all standing water from the site, due to the recent rainfall event. He placed a hose in the ponded water were I recorded the (1,200) reading and began to remove the water. |

2748928 | Primary Facility | 2018-06-20 | Routine/Complete Inspection | No Violations Noted | I arrived at the site at 13:16. Personnel from J.P. Reilly and Associates were departing from the site as I arrived. The weather conditions are partly sunny skies with light winds and temperatures in the mid 70's. There was no activity or additional personnel at the site during the inspection. A service rig recently conducted work-over operations at this site similar to the operations conducted at the BKV Ruark South and Macialek pads. For additional information regarding the work-over operations, refer to the Department's 6-13-18 inspection reports for the Ruark South and Macialek pads, inspection ID #'s 2743281 and 2744324. The rig has moved to the nearby BKV Procter & Gamble (1) pad. The secondary containment and frac tank as part of the work-over operations remain in place on this pad. The wells are in production. I used a blue Hanna HI 9831 Soil Test Electrical Conductivity (EC) & Temperature meter, and white Hanna HI 9831 Groline Soil Test meter to conduct field screening of specific conductanc |
2755659 | Primary Facility | 2018-07-10 | Routine/Complete Inspection | No Violations Noted | I conducted the inspection accompanied by Jon Sassi (EPS). There was no activity occurring at the site. If you have any questions concerning this inspection, please contact me. |
