2237949 Primary Facility 2013-01-21 Drilling/Alteration No Violations Noted Precision 528, Comp Man - Jay Lewis, @ 10433'
2272043 Primary Facility 2014-05-14 Routine/Complete Inspection No Violations Noted All wells at the P&G location location have been hydraulically fractured. Frac plugs have been drilled out for the Prevost 1H and Ruark 4H wells. Flowback procedures are set to commence shortly for the Prevost 1H and Ruark 4H wells. I recorded (1,300) microSiemens/cm conductivity in water on the frac tank battery containment liner behind the frac tanks in the northwest corner of the Well Pad. A vacuum hose to remove standing water on the surface of the Pad and containment areas of the site, was staged on the frac tank battery containment liner below the site perimeter earthen berm. The liner extends to the top of the berm. There were some tears observed in the liner, but mainly on the liner on the top of the berm, not near or in the standing water.
2429761 Primary Facility 2015-11-17 Routine/Complete Inspection No Violations Noted There was no activity at the site as we arrived around 12:00. We used an Oakton MultiParameter PCSTester 35 and a Hanna Soil Test and Electrical Conductivity & Temperature meter to screen the water in the Production Fluid Storage Tanks and Gas Production Unit containment. Conductivity readings were in the range of (700-800) microSiemens/cm in water inside the production fluid tanks containment and less than (500) microSiemens/cm in water inside the GPU containment. We collected a water sample using the PADEP Bureau of Labs Standard Analysis Code 046 in the production fluid tanks containment at N-41.58295; W-076.01905. The parameters to be analyzed are listed in the Department's 11-17-15 Macialek inspection report. The intent of screening the water for conductivity and collecting the sample is to determine if the water meets the requirements of PA Title 25, Section 78.60 and Warren's SOP in the event this containment water will be discharged to stormwater controls on-site.
2430871 Primary Facility 2015-12-08 Administrative/File Review No Violations Noted Attached are the sample results of the water sample collected in the Production Fluid Storage Tank Containment Area as indicated in the 11/17/15 inspection report. The sample results were submitted with the inspection report.
