0222626651 2018-03-29 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains six 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 2600 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. 0337844835 2016-03-09 Casing String Witnessed No Violations My inspection today finds Patterson drilling rig 288 is running 5.5 casing. An 8.5 borehole was drilled to a total depth of 21,429. I witnessed the casing being landed at a depth of 21,421. Baker Hughes is onsite to perform the cementing operations. While onsite, I reviewed the cement plan.
0665845909 2015-11-30 Drill / Deepen / Reopen Violations Noted My inspection today finds the well has been plugged back after losing 9.625 casing in the 12.25 wellbore. The company plugged the well back with a cement plug. The top of casing is at 3659 based on an attempt to retrieve the casing with a center spear tool. No DOGRM personnel were notified prior to the cementing operations. I notified Ted Dubois, drilling superintendent, of the violation by phone at 2:44 pm. I informed Mr. Dubois that a timeline of events during the plug back operation, in writing, would be required to satisfy the Compliance Notice and it will be required prior to proceeding. As well as, a proposed plan to finish drilling the well will need to be submitted prior to drilling.

1449745580 2015-12-10 Casing String Witnessed No Violations I am onsite to witness the displacement of the 9.625 casing string cement. Patterson Drilling 288 drilled a sidetrack around the previous 12.25 wellbore that was lost. The side track was established at 2490. The drilling rig ran 9571of P-110 9.625 casing. I witnessed cement returns to surface and the casing integrity test.

1738110792 2017-06-14 Completion Testing No Violations During this inspection, the well was currently in the flow back phase of completion. There was piping from the well that lead to a sand separating unit and then to the 500 barrel fluid storage tanks. There was a total of ten 500 barrel fluid storage tanks on site. The tank battery on site contained four 400 barrel production fluid holding tanks and there were also gas processing units on site.
