0204976577 2016-03-04 Casing String Witnessed No Violations On this day Lightning Energy had reached a total depth of 110 with the 32 auger bit. The 26 casing was ran and cemented by Stingray Cementing using 290 sack of Class A cement. Cement returns to surface were successful.
0436521339 2017-03-27 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well site meets ORC 1509.072 with erosion control in place and vegetation established in the areas affected by the well pad construction. Addressing the well pad, the well pad has a earthen berm perimeter and 3 producing wells on the pad with said well now in production. The well head is equipped with annular monitoring along with an emergency shut down device controlled at 3 point on the well pad. On the well pad location is the production facility consisting of a total of 4 steel production tanks capable of holding 400 barrels of fluid each, the tanks are located inside a polymer lined steel containment dike area.
0694816232 2018-08-20 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 3-string casing design with 5.5-inch diameter production casing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 550 pounds per square inch, and the 13.375-inch diameter casing gauge read 50 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and four 400-barrel production fluid tanks, and a 50-barrel produced water tank shared with the other producing wells on the well pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.

All (3) are equipped with production trees with no artificial lift present and 2.875 production tubing. Annular pressure monitoring is present along with proper well identification. The production equipment consists of (3) gas processing units and a production fluid facility that housed (4) 400 barrel tanks inside a polymer lines steel containment area. Proper identification is posted at the well head and lease road entrance.

Bow spring centralizers were ran to ensure proper standoff from the well bore. O-Tex Cementing was on site to cement the casing with 870 sacks of Class H cement. Cement to surface was established.
