0015295636 2019-03-07 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with the area affected by the well pad construction restored and vegetation established, all outward slopes were stable with no movement observed. The well pad surface and lease road is stone construction and in good repair. 5 wells are producing on the pad with a second adjacent pad accessed through this pad present identified as the Valarie well pad. Proper identification is posted at the well pad entry point and on the well head. All (5) wells are equipped with a production tree with 2.875 production tubing reported to be set in the tubing hanger inside the B section of the well head. 5.5 production casing was set in the well bore. The well head A&B sections access ports have 2 diameter riser pipes installed that are equipped with valves and gauges for annular pressure monitoring. The A section well head pressure is 50 pounds and the B section pressure is 800 pounds. The line pressure on the production sales line downstream of the well head choke manifold is 575 pounds pressure. Production equipment on location consists of (5) gas processing units and a fluid storage area that consists of (3) steel 400 barrel tanks situated inside a polymer lined steel containment dike area. Fire Extinguishers and hazard warning labels are situated at all critical work areas on the pad.
0952475714 2018-08-20 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design with 5.5-inch diameter production casing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 90 pounds per square inch, and the 13.375-inch diameter casing gauge read 55 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and four 400-barrel production fluid tanks, one of the 400-barrel production fluid tanks appeared that it had collapsed slightly it was not leaking at the time of my inspection, and a 50-barrel produced water tank shared with the other producing wells on the well pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date
1308477664 2017-03-27 Casing String Witnessed No Violations On site inspection to witness the Surface casing cement job with Sidewinder rig 103 on location along with Halliburton Cement Service. The rig has drilled a 24 diameter surface borehole to a measured depth of 680 with a coal seam being encountered at 635 to 649 with no void reported. The rig then ran 608 of 18.625 diameter buttress thread, J-55 grade casing with a float shoe and collar casing equipment ran and proper centralizer placement demonstrated, 2 cement baskets were also ran. The drilling rig loaded the hole with fresh water and circulated the last casing joint in the hole. The casing was cemented by Halliburton Cement Service with 25 barrels of 6% gel spacer pumped followed by a 10 barrel fresh water spacer and then the cement mix was pumped consisting of 204 barrels of class H cement that contained 965 sacks of cement weighing 15.8 pound per gallon by weight of scale. During the displacement cement circulation was lost at 110 barrels into displacement out of a total of 195 barrels. Displacement pressure was 240 psi prior to landing the plug and the casing was pressure tested to 740 psi upon the landing of the plug. 1 tubing was ran down between the surface and conductor to a depth of 240 and a 10 bbl cement slurry was pumped with fluid circulationg to surface. The 1st 10 barrels was allowed to sit for 1 hour before the well was topped off to surface with 75 barrels of class H cement with cement returns to surface at 63 barrels.

1752984503 2018-03-21 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 200 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 500 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
