0002946994 2004-09-01 Production Wells No Violations Follow-up revealed Mr. Miller has taken the locks off of the gate and he advised me that Mr. Seifert has removed his dozer off of his property. He also advised me that he and Mr. Seifert had to hire new attorneys due to conflict of interest. There is no change in the wells status.
0470440440 2005-05-11 Production Wells No Violations Follow-up inspection revealed Mr. Seifert has connected a yellow plastic line to the drip located to the south of the barn and burring it to the west coming out at the creek where it is rolled up and is not connected to any other gas line or gas meter. (See Photos.) WEATHER AND GROUND CONDITIONS: Sunny dry and 60 degrees. ( I contacted Tom Tugend and Jeff Fry by phone and advise them of my findings.)

1113941187 2005-04-19 Production Wells No Violations Accompanied by Jeff Fry. Follow-up revealed well not in operation and the well is not being used for domestic gas. Mr. Miller advised that there is no meter set for the sale of gas from this well, and no one from Mr. Seiferts company has been on the property since the locked gate incident. Well owner is in non compliance to Chiefs Order. Jeff took two (2) photos.
1178724906 2007-05-07 Final Restoration No Violations Inspected well location with Jeff Fry at the request of the landowner. Well owner John Seifert as well as Red Miller met us on location. We agreed that John Seifert would do the following: 1) Fix the field tile at the bottom of the hill. 2) Fix up the road by placing some diversions on the roadway. 3) Remove the larger rocks that are on the surface of the restoration area. 4) Remove the exposed electric wire from the restoration area. 5) Fill in the holes that are up on top of the hill by the old well location. John will complete this within the next couple of months when his equipment becomes available.
1596027964 2008-05-09 Final Restoration No Violations Jeff Fry and I met with John Seifert and Red Miller on location. It was determined that Seifert would apply lime and fertilizer as well as clover seed on restored area. Also, additional water bars would be added to lease road. Final Restoration not yet passed.
1642784379 2008-07-21 Final Restoration No Violations I inspected the well location and lease road on Red Millers farm. Proper grading of the well site has been completed. John Seifert has spread lime and fertilizer on the seeded well location. Vegetation is flourishing on the well site. John also has constructed water bars on the lease road to control erosion. All oil field debris has been removed. Final Restoration passed.

1850848148 2007-10-16 Final Restoration No Violations I inspected the well locations with Jeff Fry. There was significant erosion on the lease road at the top of the hill. The water shed bars that were built into the lease road have helped, however there has been some significant erosion taking place. Grading, Seeding, and Mulching of the area need to take place. There is also a pile of rocks on the hillside that need to be removed. Jeff called John Seifert and discussed what needed to be done. He said he would get to it soon and call when he is ready.
