0350934400 2009-11-16 Production Wells No Violations -On site for status check inspection for NOV issued 11/8/04 for well incapable of production and well not labeled. Talked with Mike Storms by phone on 10/16/09 regarding this & other wells on the Putoff property. Notified Mike that no production has been reported since NOV issued. Also requested he identify the wells on site. Mike indicated that all but 2 of the wells were operational but he wanted to get them labeled. Site inspection 1 month after call to allow for harvesting of crops. Well not label. Jeff Putoff indicated only well 2-0029 and 6-0003 were operational. Will contact Mike Storms regarding this & other wells on the site. It does not appear that this well is connected to the 2 tanks nearby.
0761714668 2004-11-04 Production Wells Violations Noted -On site with Jeff Fry for inspection/meeting with Paul & Mike Putoff. Paul wants the wells on the property to be either produced or plugged. Paul has not received royalties for the wells since about 1999. Weather conditions did allow for upclose inspection of the well. Will follow up later. 11 bbls of oil reported produced in 2002. Paul indicated only well # 20029 is attached to the tanks on the property.
1145382281 2006-04-11 Production Wells No Violations -On site for complaint follow-up from Paul Putoff. Met with Paul & Jeff Putoff and well owner Mike Storms. Storms is on the site getting the wells back into service. Storms has recently installed a new motor on the well. Photos taken of the well. Requested Mike to ID the well. N.O.V. # 761714668 was issued 11/8/04 for a well incapable of production. Requested operator to put the well into production and report the production. Paul Putoff told Mike to put the wells on his property into production or plugged and if he did not do either of those options, he would take Mike to court for failure to follow the lease agreement. Notified Mike that if that occurred, he could only be ordered to plug the wells and not produce them. Paul indicated that he would help Mike pump the wells if he got them into production. Some lines from some of the wells need repair. Notified Mike that N.O.V. is outstanding for this well.
1287439290 2010-04-06 Production Wells No Violations -On site for status check inspection for N.O.V. issued 11/8/04 for well incapable of production & well not labeled. Talked with Mike Storms by phone on 10/16/09. Since that time, the well has been labeled but the entire API # not listed. On 43 listed. Need to include 34-149-20043-00-00. Well not producing and Paul Putoff said it was not operational. Will check with Mike Storms to request the label be revised and check on production.
